Attestations for Living Individuals
Our staff will search for records of baptism, marriage, and church membership for living individuals. The request must be from the individual or their legal representative.
Information on deceased persons is only provided when necessary for the settlement of an estate or survivor benefits. If you are conducting genealogical research on a relative or another individual, please refer to our Genealogy Record Search.
Please note that we do not have a centralized index of names in Presbyterian records. In order for us to conduct a successful search, please provide us with the information specified in the request form below. The name and location of the associated congregation is required for all requests.
Occasionally, there are discrepancies between what you expect to find and the information as it was recorded, such as a given date or the spelling of a name. We will transcribe the information exactly as it was entered into the official records. We are not authorized to change information.
If we locate a record, we will mail a letter to the address provided below reporting the information found and bearing the seal of the Presbyterian Historical Society. If we cannot locate a record, we will notify you in writing. Our letter of attestation is generally acceptable to church and legal authorities. We do not make photocopies of the actual records due to their unique and fragile nature.
